Snaggy, regardless of what was said above, at this point what do you think of the probably of a working vaccine? Time line?

Almost all the vaccines globally are focused on creating strong antibody to Spike Protein. The virus doesn’t seem to have the ability to mutate that protein much and still function to bind well to the ACE2 receptor. Keeping it simple, avoiding a more complex antigen presentation like including nucleocapsid antigen, for example, may have advantages.

I feel like strong antibody to Spike Protein should work pretty well, and the organism will have trouble mutating away from the antibodies, and if it does, we can reload our vaccines with new antigens pretty fast.

I think there could be an effective vaccine in broad but not full distribution by third quarter.

I’m pretty optimistic but I could be completely mistaken.

Interesting thing, yesterday the POTUS said that he felt great.

When I was taking Dexamethasone, I always felt great. Incredible. Nothing hurt anymore.

Plenty of people are not going to realize just how much $$$$$ in taxpayer dollars got spent in just a few days and how many treatments that they have little chance of getting access to would have been administered.

I think Gilead charges about $3500 for the full course of Remdesivir. The supply is not unlimited, but any patient going to a hospital with an O2 sat less than 94% has a pretty good chance of getting it. The REGEN-COV2 would have been free as it's not approved yet. Trump is the best advertising Regeneron could possibly have too. Dexamethasone, you've spent more on a beer. I bet REGEN-COV2 gets an EUA soon and is very successful until it's supplanted by vaccines.

Whatever else this was, it wasn't that expensive strictly from the standpoint of Trump's medical costs.

The recklessness is unbelievable though. A lot of his inner circle is sick, and some of the senators he wants for the SCOTUS fight are ill. There's not much information coming from Senator Mike Lee's camp since he announced his positive test results 3 days ago, makes you wonder if he'll be able to vote. Ever.

I get the sense that absolutely every treatment that showed any promise has been administered.

And I agree with the recklessness, especially the message that it isn't anything to be afraid of. How many people have the resources that a president has at their disposal with total around the clock commitment?

Except for the Regeneron REGEN-COV2, Trump's treatment wasn't that unusual.

He tested positive, and isolated at home. His O2 sat dropped below 94%. At that time, the standard of care is hospitalization and supplemental O2. I'm sure he didn't want to go. Once he's hospitalized, on O2, then Remdesivir is indicated. Standard procedure.

Dexamethasone use doesn't have such strict protocols. Some evidence of lung damage would probably be good enough. That would include some markers of inflammation like C-reactive protein. Low lymphocyte counts would be a concern, some other values too. Imaging would be ordered, which is moving away from plain x-rays, toward CT scans and even lung ultrasounds, which can be done quickly at the bedside, without the radiation exposure of a CT, and can follow progression of lung involvement. I think the dose is 6mg twice a day, less than a neurosurgeon might use. The lab and imaging data hasn't been released and the TV doctors are having fits about it.

He didn't get convalescent plasma. That's an effort to give antibodies to someone but it isn't really possible to know how much of the antibody is really going to be strong and protective.

The Regeneron product is even better than convalescent plasma, we hope. It has 2 distinct antibodies, handpicked for strong binding to the Spike Protein. Didn't seem to do Trump any harm, he's doing great. The one dose he got would be considered something like a 2-4 week supply, and not meant to be repeated. This drug would be incredibly difficult for an average person to get right now, though I suspect some other rich or important people in the world could get it. Turns out the CEO of Regeneron has known Trump for years from the golf links.

That doesn't bother me. A world leader probably should have early access to a good drug.

The timing is interesting if you compare it Herman Cain's timeline.

7/2: Tests positive for Covid-19
7/10: Says he’s improving
7/15: Says his doctors seem happy
7/27: Says he’s really getting better
7/30: Dies

The last I saw of Trump was his labored breathing when he returned at the White House, but today tweets that he's feeling great! :party
