plus gauze between the toes (don't ask me how I know) is pretty much all you can do. Visiting the hospital or a doc is pointless.1. Medical tape to the neighboring, unbroken toe for support. 2. Painkillers.

I remember the first time I broke my ribs 25 years ago and went to a doc-in-the-box. They charged me $300 for taking an x-ray, a support wrap and writing a 'script. I know when I've puntured something internally. I've broken my ribs and collar bones since then, and all I do now is hit up the supermarket for a Ace bandage and booze. :laughing
Big toe sprains/breaks are a painful bitch!
However, unless it's jutting off at an odd angle, taping it to its neighbor should be okay. If it is at an odd angle, you should probably see a doc (big toes are key to being able to walk; those that have had them amputated need special weights to be able to balance, IIRC.)
